Take the First Step to a Healthier Lifestyle...
...and earn $150
Get started on a path to wellness! Take the pledge and discover the benefits of wellness through the Lifestyle Returns SM program. Saint Francis University and Highmark Blue Cross Blue Shield are offering you the opportunity to “Have a Greater Hand in Your Health.”® Lifestyle Returns is an easy-to-follow program that identifies health and lifestyle areas you need to improve or maintain. Lifestyle Returns helps you develop healthier habits and provides you with the tools and resources necessary to enhance wellness.
Complete the following five steps between February 1 and October 31.
1. Take the Online Pledge - Enroll and accept the pledge to complete the program requirements via the member Web site, www.highmarkbcbs.com.
Detailed instructions (pdf). First-time users click “Members” and then click “Register Now” from the Login box to create a User ID and password. Complete the registration form and click “Submit.” After you click “Submit” a message will appear regarding your Personal Identification Number (PIN). You will receive a PIN in U.S. mail within 7 to 10 days. You need only enter the PIN one time to access Lifestyle Returns. If you already have a User ID and password this message will not appear.
To enter Lifestyle Returns, click the “Your Health” tab and then click “Lifestyle Returns.” After you take the pledge, you will receive ongoing encouraging e-mail reminders until you fulfill the program’s requirements.
2. Complete the Online Wellness Profile – The next step to a healthier you requires completion of a 20 to 30-minute confidential Wellness Profile. The Wellness Profile is a comprehensive survey that covers all aspects of your health, including fitness, heart and general health, nutrition, substance abuse, safety and stress. When you have finished with the Wellness Profile, you will receive an immediate response with program recommendations addressing your specific needs.
3. Get Preventive Care – Schedule and/or document preventive exams and screenings that fall within your age and gender requirements during your health care benefits year. Need assistance? Go to www.highmarkbcbs.com and review the Preventive Care Schedule with guidelines for you and your family. When you have completed these exams, log on and document your visits and results.
4. Participate in Health and Wellness Programs – Part of the program requirements include participating in at least two recommended Health and Wellness Programs. Online programs cover relevant topics, such as nutrition, smoking cessation, weight control, stress management and managing various chronic conditions. In addition to the online programs, we are offering many programs on campus such as Eat Well For Life I and II, Discover Relaxation I and II, HOPE (Highmark's Osteoporosis Prevention and Education Program), Clear the Air (smoking cessation), Personal Nutrition Coaching, and The StrongWomen Program. You also have the opportunity to open a dialogue with a Blues On CallSM Health Coach who can assist you with your specific concerns. Contacting a Blues On Call Health Coach fulfills one of the requirements of Step 4, as does scheduling a flu shot.
5. Get a Healthy Education – Take full advantage of Highmark’s exceptional range of online Health Information and Tools. Lifestyle Returns requires that you access at least one online resource and then complete the follow-up survey about the tool you used.
Lifestyle Returns helps you improve your wellness and may enhance your lifestyle. Additionally, upon completion of this program, you will receive the following rewards from Saint Francis University: $25 after completing Step 2; $25 after completing Step 3; $75 after completing Step 4; and $25 after completing Step 5.
